Please call us at 909-870-5200 to schedule an appointment.
Please note that we have a 15 minute grace period for you to check in to your appointment. At that point, your appointment is considered lost as we will try to accommodate another patient who needs help. If you cannot make it, please call us as soon as you can to inform us so that we can reschedule you into a better time slot.
Of course we do. Please schedule an appointment with any of our providers and we can discuss your medical issue and place the appropriate specialist referrals where needed.
We have an after hours call service. So if you call, an on-call physician will try to assist you. Please make note, after hours service is for medical advice only. It is not a refill service. Please call during regular hours if you need your medications refilled.
